Position Summary

Under the clinical supervision of the Medical Director, the Community Health Center Clinical Nurse Manager oversees the clinical activities at the Milbank Health Center, including monitoring the care of children in foster care. The CHC Clinical Nurse Manager oversees the Milbank nursing team to assure that the clinic is operating efficiently and that services provided are in accordance with accepted standards of care and all regulatory requirements are met. In addition, the Clinical Nurse Manager monitors the panel of children in foster care and coordinates with office administrative staff to assure that children in foster care are receiving the care that they need. The position combines leadership, organization, and clinical skills and requires someone fluent in each of these areas.

Essential Duties

Patient Care and Oversight of Clinical Operations:

  • Provides daily clinical support to other provider staff, including nursing assessment and treatment.
  • Provide nurse assessment of sick patients and triaging those patients in person and on the phone to determine the appropriate level of care needed.
  • Ensures an adequate patient flow system, making sure that walk-in patients and appointments are seen in a timely manner.
  • Shows competency in nursing practices: proper nursing skills, proper use of medical equipment and terminology, employs current nursing protocols and procedures.
  • Develops nursing assessments of patients by obtaining the child's comprehensive birth, developmental, health, immunization, and social histories.
  • Provides hearing and vision screening of NYS mandated services to identify potential problems and completes appropriate referrals in a timely manner.
  • Assists NP/MD with physical exam as needed.
  • Dispenses medication as directed.
  • Provides triage and first aid treatment for minor injuries and illnesses.
  • Refers child to appropriate provider when a health problem requiring medical treatment is identified and follows-up.
  • Provides family health education such as self-care, nutrition, parenting, and family planning.
  • Provides health education and skills building for foster and biological parents on the child's medical condition, proper administration of medication, safety precautions including response to side effects, universal precautions, age anticipatory guidance, and any other training based on the care plan.
  • Triages daily phone calls, and turning such calls into tele-visits when applicable.
  • Provides wellness checks and phone follow-ups for patients who were seen in the Emergency Room (ER) or Urgent Care.
  • Provides nursing case management for patients, including those with acute or chronic illnesses or conditions.
  • Assists with Health Connection (HC) issues presented to providers.
  • Follows up on all outgoing foster care referrals on a weekly or biweekly basis.
  • Implements policies and procedures for the maintenance of clinic pharmacy, including the emergency kit, immunizations, onsite laboratory testing, phlebotomy, and specimen preparation for lab pick-up, infection control, and other regulatory requirements as assigned.
  • Supervises and evaluates Licensed Practical Nurses, medical assistants, and other nursing staff at the Milbank clinic.
  • In the event of staff shortages, may be asked to be available for coverage at school-based health center.

Administration:

  • Leads daily clinical staff meetings (huddles).
  • Participates in Children's Aid CQI committee and oversees implementation of CQI projects at Milbank.
  • Collaborates and meets at least weekly with the Milbank Clinical Administrator and Director of Mental Health Services.
  • Oversees schedule for nursing staff to assure adequate coverage during sickness and vacation leave.
  • Coordinates with the Bronx CHC clinical nurse manager to coordinate consistent nursing practice throughout Children's Aid community clinics and to address scheduling challenges between the two nursing staffs.
  • Attends clinical meetings, as well as other Children's Aid site meetings as required.
  • Responsible for keeping Medical Director and Clinic Administrator updated on current issues and/or problems related to the clinic's operations.
  • Responsible for carrying out agency's policies and professional standards.
  • Leads the recruitment, interviewing, selection, and processing of nursing staff, in collaboration with Medical Director.

Interdisciplinary Skills:

  • Works collaboratively with nurses, clinicians, health educators, social workers, administrative staff, foster care case planners, and public health officials to assure the safe coordination of care for patients and their families.
  • Shares responsibility with clinic leadership to manage vacancies and time off, including responding to messages and phone calls related to patient care or clinic operations.
  • Participates in agency seminars and training programs.
  • Assists with scheduling of appointments.
  • Performs other related duties as required.
Skills and Qualifications:
  • Baccalaureate Degree preferred.
  • Three years in community health, child health, or related field.
  • Current CPR certification and First Aid training.
  • License to practice as a Registered Professional Nurse in New York State.
  • Impeccable organization and attention to office details.
  • Experience in electronic health records.
  • Facility in electronic communication and documentation, including e-mail, monitoring of and documentation within spreadsheets, use of business-oriented social media.
  • Excellent verbal and writing skills.
  • Bilingual English/Spanish preferred.
  • Flexibility and ability to work under pressure.
