Registered Nurse - Services for the Underserved : Job Details

OF ROLE:The Registered Nurse collaborates with physicians and the multidisciplinary team to ensure residents receive timely care that meets evidence-based practice standards. The role integrates utilization management, care coordination, and discharge planning functions to assist residents in achieving positive clinical outcomes and safe transitions to the community. The Registered Nurse provides resident/family support and education and maintains strong relationships with community providers. ESSENTIAL DUTIES & RESPONSIBILITIES:Implement and establish plan of care in accordance with scope of practice in collaboration with the physician, clinical team members, social services, physical therapy, the resident and the resident's family by tailoring care to meet individual needs of the resident. Monitor resident's healthcare/medical needs and provide coaching, training and feedback, to assists in daily management of the residents.
