License: Must have license in hand, no exceptions. Certifications: CPR, BLS Must-Have: - Recent specialty experience is required. - Proficiency in OASIS and HomeCare HomeBase (HCHB) EMR systems, having used them within the last 6 months, or must complete a retraining course. - Must be open to traveling up to 75 miles from the assigned care center. - Must have reliable transportation, a current driver's license, and agency-required liability insurance. Description: Provide and coordinate home nursing care according to physician's orders. Responsible for applying the nursing process competently. Directly supervise and evaluate Home Health Aides and LPNs/LVNs. Complete documentation within 24 hours of patient appointments. Accept every patient scheduled regardless of location. Must be comfortable with travel that may exceed 20 minutes between patients. There are no guaranteed hours; expected to reach weekly productivity points equating to 40 hours per week. Floating to another clinic may be an option if patient census is low.
