SUMMARY

As a Registered Nurse in Home Care, you will play a vital role in promoting and maintaining the health of individuals in the community. This position involves providing teaching, counseling, and essential preventive and rehabilitative services. You will be responsible for assessing health needs and collaborating on care plans for individuals and families.

ElderONE

ElderONE, an affiliate of Rochester Regional Health, is a Program of All-Inclusive Care for the Elderly (PACE). We serve seniors in Monroe, Ontario, and Wayne counties, offering a progressive alternative to long-term nursing home placement. Our program empowers seniors aged 55 and older with chronic care needs to maintain independence in a safe, comfortable home environment.

RESPONSIBILITIES

• Patient Care: Perform initial assessments; establish care plans and provide prescribed treatments; coordinate the delivery of comprehensive 24-hour care.

• Communication: Counsel and guide individuals and their families towards self-help by addressing physical, emotional, and environmental challenges.

• Evaluation & Documentation: Regularly reassess each patient's condition; maintain accurate clinical documentation; supervise and support home care staff.

Required Qualifications:

Registered Nurse license in New York State.

Valid NYS Driver's License.

Preferred Qualifications:

1 year of nursing experience preferred.

Prior home health, clinical, and patient care experience preferred.

EDUCATION:

AS: Nursing

LICENSES / CERTIFICATIONS:

BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

$73,528.00 - $98,072.00
