FlexStaff is seeking a dynamic Clinical Manager of Integrated Care on behalf of our client, to spearhead seamless care transitions from hospital to home, reducing readmissions and elevating health outcomes for our participants.

Responsibilities:

  • Lead Care Transitions: Collaborate with discharge planners, physicians, and care teams to craft personalized, effective discharge plans. Ensure participants understand their care instructions and medication management.

  • Assess & Coordinate: Conduct in-home assessments, monitor health during transition, and adjust care plans to meet individual needs.

  • Provide Skilled Nursing: Deliver expert nursing services, including medication administration, wound care, and vital sign monitoring, while educating participants and families for better health management.

  • Supervise & Support Staff: Mentor nurses, PCAs, and HHAs, conduct joint visits, and uphold clinical standards to ensure top-quality care delivery.

  • Ensure Quality & Compliance: Maintain meticulous documentation, participate in quality improvement initiatives, and stay current with regulatory standards.

  • Empower Participants & Families: Educate on lifestyle modifications, safety, and health management, fostering independence and confidence.

Qualifications:

  • Proven experience in clinical care, home health, or care coordination

  • Ability to lift up to 50 pounds and perform physical tasks required for patient care

  • Strong decision-making, analytical, and communication skills

  • Willingness to travel between participant homes and work in diverse environments

  • Passion for improving patient outcomes and team leadership

The salary range for this position is $135,000-$140,000/year
