We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support. Your expertise in managed care, patient assessment, and medical terminology will be essential in ensuring the well-being of our patients. Key Responsibilities:
- Provide direct nursing care and clinical oversight for individuals with developmental disabilities in accordance with OPWDD guidelines.
- Administer prescribed medications and treatments, monitor for side effects, and maintain accurate medical records.
- Conduct health assessments and respond to medical emergencies as needed.
- Collaborate with interdisciplinary teams, including DSPs, medical providers, therapists, and family members.
- Develop and update individualized nursing care plans and contribute to person-centered service planning (ISP).
- Educate staff on health-related issues, proper medication administration, infection control, and other medical protocols.
- Administers medications when appropriate, and supervises AMAP certified staff in medication administration. Supervises all medication prescribed for individuals in accordance with agency policies.
- Provides supervision and training for Licensed Practical Nurses and/or less experienced.
- Provides immediate first aid with referral to outside emergency services or private physician.
Experience:
- Minimum of 1 years of experience as a Registered Nurse in a clinical setting
- Experience in managed care or case management is highly preferred
- Strong knowledge of medical terminology, physiology, and medical records management
- Ability to assess patient needs and develop comprehensive care plans
- Experience working with diverse patient populations, including pediatrics and behavioral health
- Familiarity with emergency room protocols and procedures
