Summary:
The Neuroscience Registered Nurse Care Coordinator is a leadership role within an interdisciplinary team performing transitional process navigation to influence optimal clinical outcomes. This is achieved by identifying the patient population to ensure effective transitional care by performing real time audits and care transition, tracking proactive care to at risk patients following an acute neurological event to reduce the burden of illness, improve compliance with evidence based clinical guidelines, care protocols/care pathways, the coordination of care, resources and services across the continuum while reducing the total cost of care and improving the patient experience of care. This role provides direct clinical support to CHS and Trinity Ambulatory Neurology offices including phone triage, patient assessment, patient education, care documentation and disease management activities under the supervision of Physicians and/or Mid-Level Providers.
Responsibilities:
EDUCATION
Graduate of an accredited school of professional nursing, BSN preferred
Current, unrestricted licensure by New York State as a Registered Nurse (RN) required
Current BLS certification
Current ACLS, or completion required within 3 months of employment
Current NIHSS Certification
EXPERIENCE
One (1) year experience with an electronic medical record (EMR)
Two (2) years' experience in outpatient neurosciences
KNOWLEDGE, SKILL AND ABILITY
Knowledge of prior authorizations for medications, surgeries, and imaging
Knowledge of basic neuro pathology
Excellent organizational and time management skills
Ability to set priorities when organizing care for patients with varying acuity
Strong telephone and technology navigation skills
Collaborative teamwork abilities
Ability to function autonomously using the nursing process
Strong problem-solving skills, excellent oral and written communication skills
Attention to detail, and a high degree of professionalism
