Now Hiring: Registered Nurse (RN) - Community Health Assessor

Adecco Healthcare & Life Sciences is seeking skilled Registered Nurses to perform UAS-NY assessments across Brooklyn and surrounding areas. This flexible, high-impact role allows you to build your own schedule while supporting adults, seniors, and individuals with disabilities in accessing long-term care services.

Locations : Bronx, NY (University Heights, E Tremont, Morrisania, Yankee Stadium, Fordham-Belmont) P ay Rate: $55-$60/hr (Based on experience, education, and location)

Flexible Scheduling Options:

Extended hours: Monday-Sunday, 7:30 AM - 7:00 PM

Create your own weekly schedule

Make up time for interrupted or no-show appointments

Opportunity to work any calendar day

Qualifications:

Active New York State RN License (REQUIRED)

UAS-NY or OASIS Community Assessment Tool experience (REQUIRED)

Minimum 2 years of in-home care experience (within last 15 years)

Skilled in working with adult/geriatric populations and individuals with disabilities

Proficient with computers and telehealth assessments

Familiarity with long-term care services & community resources

Willingness to travel locally as needed

What You'll Do:

Conduct comprehensive Community Health Assessments to support care planning

Perform face-to-face follow-ups to ensure care coordination

Connect individuals with appropriate long-term care services

Identify care gaps and support clinical quality standards

Educate consumers and caregivers while contributing to special initiatives

Pay Details: $55.00 to $60.00 per hour
