Registered Nurse Assessment Coordinator (RNAC) – Full-Time

Are you a detail-oriented Registered Nurse with a passion for care planning and compliance? We're looking for a Registered Nurse Assessment Coordinator (RNAC) to join our skilled nursing team in Pennsylvania. In this key role, you'll lead the coordination of resident assessments, ensuring accurate documentation, compliance with federal regulations, and excellence in resident care.

Key Responsibilities:
  • Coordinate and manage the completion of resident assessments (MDS) and care plans in compliance with federal and state regulations

  • Work closely with the interdisciplinary team to ensure timely and accurate documentation

  • Monitor quality measures and identify opportunities for improvement

  • Educate staff on care planning processes and best practices

  • Participate in resident care conferences and audits as needed

  • Ensure assessments reflect resident needs and services provided

Qualifications:
  • Current RN license in the state of Pennsylvania (required)

  • Experience with MDS and care planning in a skilled nursing or long-term care facility (required)

  • Strong organizational and communication skills

  • Experience with PointClickCare or similar EHR system (preferred)

  • Knowledge of PDPM and reimbursement processes (preferred)
