Title: Registered Nurse ACT Team NYC – 1 day hybridWe are seeking a compassionate and skilled Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team.This role involves providing holistic, person-centered nursing care to individuals with serious mental illness (SMI) and co-occurring disorders in a community-based setting.The ideal candidate is passionate about working with underserved populations and thrives in a mobile, team-oriented environment.Schedule: M-F, 09:00am-05:00pm with one hybrid dayLocation: Manhattan ESSENTIAL DUTIES & RESPONSIBILITIES:Provide medication education, administration, and monitoring to ACT clients.Conduct comprehensive nursing assessments, focusing on both physical and mental health needs.Work closely with psychiatrists, therapists, case managers, and peer specialists to develop and implement individualized treatment plans.Support clients in managing chronic health conditions and accessing medical care.Offer health education on topics such as medication adherence, nutrition, diabetes management, and harm reduction.Participate in daily ACT team meetings and interdisciplinary care planning.Respond to crises and provide on-call support as needed.Ensure compliance with state and federal regulations, maintaining accurate clinical documentation.REQUIRED EDUCATION AND EXPERIENCELicensed Registered RN within the state of New York.Must have excellent verbal, written and interpersonal communication skills.PREFERRED QUALIFICATIONS & SKILLSExperience working with behavioral health and substance abuse clients ideal Experience working in ACT, mobile crisis, or forensic mental health settingsBenfits: Weekly payGuaranteed Hours401k retirement planSick payReferral bonusMedical benefitsDental benefitsVision benefitsSalary : $50-55/hrPlease Email: #INDALL #LI-SF1
