Location: Bronx & Queens, NY Schedule: MondayFriday, 8:30 AM 5:30 PM (40 hours/week) Type: Full-Time | Field-Based | RN Required

What You'll Do As a vital member of our care team, you will:

• Travel to patient homes to provide expert clinical services, including wound care and medication administration.
• Conduct comprehensive health and functional assessments.
• Develop and implement individualized care plans in collaboration with physicians and care teams.
• Educate patients and caregivers on condition management, medication adherence, and home safety.
• Use your clinical judgment to monitor patient progress and escalate concerns as needed.
• Partner with care coordinators and community organizations to ensure holistic support.
• Document all care activities accurately in our electronic health record system.

What You Bring

• Licensure: Active, unrestricted RN license in New York State
• Education: Graduate of an NLN-accredited nursing program (BSN preferred)
• Experience: 3+ years of clinical nursing experience, with 12 years in home care or a similar setting preferred
• Skills: Strong clinical assessment and critical thinking, Excellent communication and case management abilities, Proficiency with EHR systems
• Other: Valid NYS driver's license and reliable transportation preferred, Willingness to provide occasional weekend or evening coverage

Pay and Benefits The pay range for this position is $120000.00 - $125000.00/yr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
