Job Summary The Quality Coordinator is responsible for monitoring and pursuing CMS STARS HEDIS measures, conducting patient outreach calls, and collaborating with center leadership to achieve STARS and HEDIS objectives. This position is part of the market operations team, working with various payors within an outpatient senior-focused primary care setting.
Key Responsibilities - Conduct enterprise and local strategies for closing STARS gaps.
- Review medical records to document and close Focus On Care measures.
- Request and upload medical records into the electronic medical records system.
- Perform patient outreach calls to support quality gap closure, assisting members with scheduling and completing screenings.
- Schedule and perform ancillary visits to complete in-center screenings.
- Meet regularly with Market Leaders to discuss progress and strategize initiatives.
- Ensure compliance with OSHA safety standards and maintain legal, and ethical documentation.
- Additional responsibilities and duties as needed.
Qualifications - Working knowledge of quality measures and quality gap closure strategies.
- Experience working in a primary care setting.
- Proficient with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, Teams).
- Demonstrated team building skills.
- Ability to work independently with attention to detail.
- Sensitivity to the needs of culturally diverse staff, patients, and communities.
- Maintains HIPAA standards and confidentiality.
Experience - A minimum of 1 year work experience as a Medical Assistant.
- Specific experience with CMS STARS program (HEDIS) preferred.
- Experience with EClinicalWorks or similar EMR.
- Experience working with geriatric patients is a plus.
Education - High school diploma or equivalent education (GED) required.
- Graduation from a nationally accredited Medical Assistant program OR comparable healthcare vocational training required.
- BLS for Healthcare Providers required.
