In addition to behavioral health care, Metrocare provides primary care centers for adults and children, services for veterans and their families, accessible pharmacies, housing, and supportive social services.

The Qualified Mental Health Professional working in the TCOOMMI program will provide intensive, community-based, therapeutic intervention and supervision to adults and/or youth on probation. Services also include monthly medication monitoring with a psychiatrist, psychosocial/rehabilitative services, assessment, development of measurable treatment goals and objectives, referral, linkage, advocacy, monitoring, crisis intervention, and continuity of care. The overall goal of this position is to maximize the consumer's potential level of functioning, reduce hospitalization, and aid in the successful reintegration of consumers into the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Identify and assist consumers in obtaining entitlements by providing referrals, advocacy, and negotiation, as needed.
  • Co-facilitate family education workshops.
  • Provide individual, family, and group rehabilitation sessions. Evaluate progress of counseling sessions, solicit feedback from consumers, and consult with colleagues and team leaders when dealing with unfamiliar/uncomfortable issues.
  • Facilitate inpatient admission upon request of the treatment team. Provide continuity of services throughout inpatient stay. Participate actively in discharge planning. Meet with the consumer within 5 days of discharge into the community.
  • Provide crisis intervention during the work week. Make home and hospital visits, initiate mental illness warrants, and work with police and other public servants as needed to address crises.
  • Follow-up within 24 hours on assigned consumers who access emergency services. Report status-disposition to the treatment team. 100% compliance expected.
  • Oversee and assist assigned consumers with medication adherence. Provide medication training, arrange transportation to scheduled office-based appointments, and prompt needed refills.
  • Follow-up with assigned consumers who miss a scheduled clinic appointment. Document attempts to contact consumers and report repeated unsuccessful efforts to contact the treatment team.
  • Participate in interdisciplinary team staffing. Present psychosocial assessment findings. Encourage consumers to attend staffing. Update the team on progress or lack of progress in reaching agreed-upon goals.
  • Review service plans every three months or as clinically indicated. 100% compliance expected.
  • Develop measurable objectives and goals agreed upon by the consumer.
  • Formulate individual service plans based on assessment findings for all new admissions. 100% compliance expected.
  • Complete a psychosocial assessment on each new admission as clinically indicated. 100% compliance expected.
  • Provide overall service coordination, referral, linkage, and advocacy to a caseload of up to 25 intensive consumers with varying needs. Perform duties in concert with other members of an interdisciplinary treatment team.
  • Perform other duties as assigned.

QUALIFICATIONS

EDUCATION AND EXPERIENCE:

  • Bachelor's degree in human services with 1 to 3 years of experience in MH/MR or a related field, or a Master's degree with 0 to 2 years of experience in human services.

MATHEMATICAL SKILLS:

  • Basic math skills required.

REASONING ABILITY:

  • Ability to successfully use an automated clinical record-keeping system.
  • Able to work flexible hours and independently with minimal supervision.
  • Ability to organize and prioritize tasks.
  • Effective written and verbal communication skills.
  • Able to maintain work in 95% compliance with standards at all times.

  • Ability to apply common sense understanding to carry out simple one or two-step instructions.

COMPUTER SKILLS:

  • Ability to utilize the internet for resources.
  • Use computer, printer, and software programs necessary for the position (i.e., Word, Excel, Outlook, and PowerPoint).

CERTIFICATIONS, LICENSES, REGISTRATIONS:

  • Liability insurance required if the employee will operate a personal vehicle on Center property or for Center business. Must be insurable by the Center's liability carrier.
  • Current State of Texas Driver License or if licensed in another state, must obtain a Texas Driver License within three (3) months of employment.
