Overview The Patient Navigator will connect patients to resources to support their physical, psychological, and social needs. This position will work with social services, health and wellness programs, and behavioral/mental health support services. They will serve as a navigator for Health District clients to navigate them towards services and resources. Responsibilities • Provide navigation to patients at the Health District and outreach events to help provide access, education, and connection to services and resources • Support In application completion and appointment scheduling • Remain up to date on available resources and processes • Communicate effectively with Health District divisions and community partners to improve the navigation of access to services and resources in the community that include social services, health and wellness programs, and behavioral/mental health support services • Document the report service and resource connections, activity results, and apply continuous evaluation and monitoring of the navigation support • Create and provide monthly and quarterly reports • Participate in outreach events including the Health Education division outreach and events • Provide information on navigation support, education on resources available, and process for connection and referral • Follow up with partners and community organizations on referral success and completion • Communicate with patients on appointments and resource connections as needed via phone or email • Build positive and supportive relationships with patients • Create and continue relationship building with community partners and organizations • Work collaboratively and effectively with internal and external partners, patients, and stakeholders Position Type and Typical Hours of Work • Non-Exempt - Full-Time • Non-rotating 40-hour work week, Monday - Friday 8:00am-5:00pm • Must be available to work additional hours as needed • Flexibility to work evenings, weekends, and holidays is a schedule requirement • In the event of an emergency, employees are required to work to provide for the safety and well-being of the public, including the delivery and restoration of vital services The above statements are intended to describe the general nature and level of work being performed by individuals assigned to this job. Minimum Qualifications • Associate's degree (AA/ AS); or 2-year technical certificate • Two (2) years of experience Licenses and Certifications Required • A valid driver's license is required; successful out-of-state candidates must be able to obtain a valid Texas driver's license within 90 days of hiring Preferred • Community Health Worker Certification Employment Testing Employment is contingent on passing any post-offer pre-employment screening as listed below: • Criminal Background Check: Yes • Motor Vehicle Record Check: Yes • Drug Screening: Yes • Physical Exam: Yes • Clearing House Query: No Basis of Rating A recruitment consultant will evaluate all applications against the posted qualifications.
