Job Description :
Determines eligibility for services by screening clients according to program requirements and guidelines. Performs general office tasks. Accurately performs data entry of program-related data and information in the Cornerstone and AS 400 computer system. Assists in educating the public, potential clients, and families, in community outreach activities. Schedules clients for IBCCP Program appointments, follow-up appointments, and tracks results and recommendations under direct supervision. Performs verbal language interpretation and written language translation as needed. Develops and maintains a basic knowledge of services available at the Lake County Health Department and Community Health Center (LCHD/ CHC). Develops and maintains a basic awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities. Develops and maintains a working knowledge of individual Emergency Preparedness roles.
