Job Duties: The successful candidate will be responsible for:
- Assessing, planning, organizing, and participating in rehabilitative programs to enhance patient mobility, alleviate pain, increase strength, and correct disabling conditions resulting from disease or injury.
- Building confidence and cooperation with patients, their families, support groups, and healthcare providers through competent assessment, attentive monitoring, effective communication, and care.
- Adhering to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering optimal patient care.
- Providing clinical supervision to assistants and aides, and serving as a clinical instructor for assigned students.
Key Responsibilities:Rehabilitation Services:
- Plan, prepare, and implement personalized physical treatment programs to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
- Complete and document patient evaluations and tests per established standards and regulatory requirements.
- Develop and update the patient's Plan of Care (POC) based on data, identified problems, and diagnosis from exams.
- Assess and provide/progress treatment for patients per plan of care per established standards.
- Complete all documentation related to charge capture per established standards.
- Coordinate patient care effectively and efficiently with other care providers and support team members.
- Work flexible shift assignments, including varying days of the week and hours.
Clerical Tasks:
- Prepare and complete accurate documentation related to patient care, including all therapy performed in medical and departmental records per standards.
- Perform general clerical tasks, such as answering phones, updating patient appointments, and managing electronic health records (EHR).
Safety and Compliance:
- Participate in quality improvement processes and interdisciplinary projects.
- Maintain a clean, neat, and safe working environment, including cleaning and disinfecting equipment after use.
- Inspect and use equipment per manufacturers' guidelines, recording data and reporting malfunctions per policy.
- Maintain strictest confidence of all patient Protected Health Information (PHI) and protect all PHI from accidental, intentional, or inappropriate disclosure.
- Prepare, administer, and document services in accordance with state and federal regulations.
- Retain certification and complete mandated continuing education requirements to meet facility and regulatory agencies' accreditation and licensure requirements.
Education: Graduate of an accredited physical therapy graduate program.
