What You Will Do
Utilizes approved assessments toidentifyclients/membersneeds and family needs; develops initial and ongoing clinical plan of care. Updates plan at specified intervals, and as needed based on changes in client/member condition orcircumstances
Performs andmaintainseffective care management for assigned caseload of clients/members. Leads the care coordination for complex psychiatric clinical cases. Tracks andmonitorsprogress;maintainsdetailed,accurateandtimelyprogress notes and other documentation
Provides supportive counseling and/or supportive therapy as well as ongoing mental healthservices
Collaborates and refers toappropriate agenciesasrequired. Addresses any client/member concerns to ensure satisfaction with overall services provided and uses motivational interviewing techniques to foster behavioralchanges
Develops inventory of resources that meet the clients/members needs asidentifiedin theassessment
Provides linkage, coordination with, referral to and follow-up withappropriate serviceproviders and managedcare plans. Facilitates periodic case record reviews and case conferences with all providers serving theclients/members
Provides information andassistancethrough advocacy and education to clients/members and family onavailability and eligibility of entitlements and community services. Arranges transportation andaccompaniesclients/members to appointments asnecessary
Assistsclients/members and/or families in the development of a sustainable network of community-basedsupports,utilizingidentified strengths and tools designed to prevent future participant crises and/or reducethe negative impact if a crisis doesoccur
Participates ininitialand ongoing trainings as necessary tomaintainand enhance clinical and professionalskills
Maintains updated case records in program EMR. Maintains case recordsin accordance withprogrampolicies/procedures, VNS Health standards and regulatoryrequirements
Participates and consults with team supervisor in case conferences, staff meetings,utilizationreview anddischarge planning meetings todetermineif client/memberrequiresan alternate level of care or isappropriate fordischarge
Participates in 24/7 on-call coverage schedule and performs on-call duties, asrequired
Acts as liaison with other communityagencies
Provides short term counseling (coping skills, trauma informed, decision making) and Risk HealthAssessment/Safety Planning
Collects and reports data, asrequiredwhile adhering to productivitystandards
Leads andparticipatesin“Network Meetings” with client, client/ member's personal support network andother team members using the Open Dialogue Model
Master's Degree in Social Work, Psychology, Mental Health Counseling, FamilyTherapyor related degree
Minimum of two years of mental health work experience providing direct services to clients/members with Serious Mental Illness (SMI), developmental disabilities, substance use disorders and/or chronic medical conditionsrequired
Effective oral/written/interpersonal communication skillsrequired
Bilingual skills may berequiredasdeterminedby operationalneeds
License and current registration to practice as a Mental Health Counselor,Marriage and Family Therapist, Social Worker, Clinical SocialWorkeror related license in New York State
Valid NYS ID or NYS driver's license may berequiredasdeterminedby operationalneeds.
