About Chronius Health

Chronius Health is revolutionizing primary care for the most overlooked patients in modern medicine. We are the first virtual (telehealth) primary care practice built exclusively for the 75 million people living with complex, chronic conditions—those navigating overlapping diagnoses, debilitating symptoms, and years of being dismissed or misdiagnosed by a fragmented healthcare system.

From autoimmune diseases and fibromyalgia to long COVID and IBS, our patients face multi-system conditions that defy traditional medical silos. Chronius is reimagining what care can look like for them: deeply personalized, relationship-driven primary care integrated with white-glove care coordination and unwavering patient advocacy. Our clinicians are not only trained in the medical complexity of these conditions, they are also backed by a mission-driven care coordination team committed to leading patients through a healthcare system that wasn't built for them. At Chronius, we're not just improving care. We're rebuilding trust, redefining access, and restoring dignity for patients long left behind.

And we're not only transforming the patient experience, we're also solving real, growing pain points for other healthcare providers, including:

  • Specialists often see complex patients whose care is fragmented, poorly coordinated, or dependent on them for more than just specialty expertise. Chronius steps in as the patient's primary care partner—managing day-to-day needs, coordinating across specialties, and ensuring continuity between specialist visits. This reduces unnecessary outreach, improves adherence to care plans, and allows specialists to stay focused on their area of expertise while knowing the patient has the support they need.

  • Primary care providers are often overwhelmed by patients whose complex conditions require more time, coordination, and follow-up than traditional models can support. These cases strain schedules, increase administrative burden, and contribute to burnout. By referring to Chronius, PCPs can confidently transition these patients to a team equipped to manage their care more intensively—without compromising quality or continuity.

About the Role

We're hiring our first full-time team member focused on provider outreach and referral network development. As we shift from proving out our care model at a small scale to rapidly expanding, this role will be critical to accelerating our reach and impact. You'll be responsible for identifying, engaging, and building strong, mutually beneficial relationships with outside providers to drive patient referrals. This is a hands-on, builder role: you'll create the playbook, design the systems, and develop the outreach materials needed to connect more patients to the care they deserve. We see this as a player/coach role in which the ideal candidate will start off creating the playbook as an individual contributor and then, over time, potentially hire a team to grow the function as we scale within Florida and then nationally. We're looking for someone energized by the opportunity to directly influence our growth trajectory and eager to spend time in the field, forging relationships in person with providers at least 4 days per week.

***Must be physically located in Florida***

Reporting to the VP of Growth and Operations, you'll also work closely with our care coordination team and our clinical leaders to create a robust flywheel for patient acquisition via a provider referral network. This includes:

Provider Outreach & Relationship Development

  • Build and manage relationships with outside primary care and specialty providers—including cardiology, neurology, rheumatology, psychiatry, gastroenterology, and allied health professionals—across Florida
  • Conduct in-person and virtual outreach to educate providers and office staff on Chronius' model, lead presentations, establish ongoing relationships, and serve as the primary point of contact for referral questions or feedback
  • Collaborate with internal teams to develop and refine provider-facing materials and messaging that support strong engagement and clear communication
  • Manage existing referral relationships on an ongoing basis to ensure continued referral flow

Referral Coordination & Operational Support

  • Educate referring offices on the referral process and ensure a smooth patient handoff, onboarding, and continuity of care
  • Partner with clinical and care coordination teams to streamline workflows and follow up on referred patients to confirm visit completion

Performance Tracking & Strategic Insights

  • Track inbound referral activity volume resulting from your provider relationship development efforts, analyze trends, and report on referral volume, outreach effectiveness, and partnership development
  • Share insights from the field to continuously improve outreach strategy, messaging, and operational processes

Who You Are

  • Physically located in Florida
  • 3+ years of experience in provider outreach-related roles such as provider referral network development, pharmaceutical or medical device sales, healthcare business development, care outreach, or partnerships within a clinical or health tech setting
  • Proven ability to engage and build trust with a wide range of healthcare professionals—from front desk staff to CMOs
  • Experience working directly with PCPs and specialists is preferred; strong understanding of primary care and chronic disease management is a plus
  • Demonstrated success in growing referral pipelines, sales territories, or healthcare partnerships
  • Familiarity with the Florida healthcare landscape and existing connections within provider communities are highly valued
  • Exceptionally organized, self-directed, and results-driven, with excellent interpersonal and communication skills
  • Energized by the opportunity to build from the ground up in a mission-driven environment focused on improving care for people with complex chronic illness
  • Based in Florida with flexibility to travel locally up to weekly and regionally up to several times per month
  • Bachelor's degree in healthcare administration, business, marketing, or a related field is preferred

Compensation & Structure

We offer competitive compensation and the opportunity to be part of a purpose-driven organization that is dedicated to making a positive impact on the lives of patients. If you are an ambitious and experienced business development leader who is passionate about transforming healthcare and creating meaningful change, we invite you to apply and join our dynamic team.

Total expected compensation for this role ranges from $50,000 to $85,000, depending on experience, and includes a performance-based bonus. We also provide travel reimbursement for in-state provider outreach activities.
