Description Compensation Range: $65,000 - $70,000 yearly commensurate with experienceBenefits: We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. Overview: The Program Coordinator (PC) is responsible for all programmatic, clinical, and operational aspects of the 55+DOHMH Scatter site housing program. This encompasses care coordination for our residents, performing quality assurance tasks through record checks and data reports, providing direct services, and ensuring that recreational activities are offered. The PC ensures resident records are in-compliance with HASA requirements.Responsibilities: Primary:
  • Oversee all programmatic, clinical, and operational aspects of the Older Adult program.
Secondary:
  • Provide documented supervision and guidance to Housing Case Managers.
Other functions required with this position include:
  • Ensuring that 55+ DOHMH Program is contractually compliant and adheres to internal as well as HASA rules and regulations.
  • Representing the programs during inter-disciplinary meetings, treatment planning meetings, and special events.
  • Monitoring residents' viral suppression rates and making appropriate referrals, as needed.
  • Facilitating monthly community meetings and ensuring an active Resident Advisory Board.
  • Reconciling petty cash fund and client and staff MetroCard's.
  • Submitting internal activity forms for new client admissions, changes in income, and discharges.
  • Meeting with residents on an as-needed basis.
  • Participating in the recruitment of qualified staff, including interviewing, and making recommendations for hire. Monitoring and assisting with the new-hire orientation processes to ensure consistency in training among all clinical providers.
  • Enforcing the Residential Code of Conduct and/or House Rules.
  • Reviewing monthly rent rolls and developing individual plans of action to address arrears.
  • Enthusiastically participating in advocacy and direct actions to end the twin crises of homelessness and HIV/AIDS.
Minimum Requirements
  • Master's degree in social work, psychology, counseling, or related field required. LMSW and/or CASAC strongly preferred.
  • Experience with and proven commitment to working with low-income and marginalized communities, as well as individuals with multiple chronic medical and/or behavioral health conditions.
  • Willingness to work occasional overtime and offer flexibility regarding scheduling work time.
  • Ability to prioritize and multi-task.
  • Dependable and flexible
  • Able to maintain a high level of client confidentiality.
  • Excellent internal, external customer service skills and strong interpersonal skills.
  • Good communication skills both verbally and written including the ability to communicate with all levels of management.
  • Ability to prioritize and multi-task.
  • Must be proficient in Microsoft Outlook, Word, and Excel.
  • Ability to work under pressure.
  • Self-motivated and self-directed with the ability to assert oneself.
