Program Manager for Support Services - Community Health Center (Jackson Heights Location)*perm Our mission is to improve the health of our community and to increase access to comprehensive primary care, preventive healthservices, mental health, and supportive services. Location: Jackson Heights, NY Pay: $58,500 - $78k DOE Schedule: Monday to Friday, 9-6p Position Summary : Responsible for the operation of direct support services programs, including healthcare case management, grant-funded case management, food and nutrition, and support groups. Qualifications:
  • BA/BS in a relevant field with 5 years of full-time experience; an advanced degree in social work, human services, mental health, or equivalent with 3 years of experience may be considered.
  • Two (2) years of supervisory experience preferred.
  • Experience working with target populations such as individuals with Serious Mental Illness, children and families, immigrants, developmental disabilities, SUDs, chronic hepatitis, LGBT, high risk or living with HIV/AIDS, and Asians and Pacific Islanders is strongly preferred.
  • Appreciation of and commitment to the center's mission and values.
  • UAS-NY certification required within 3 months of hire.
  • Excellent customer service skills.
  • Ability to work in a multidisciplinary and diverse environment.
  • Effective communication skills in English; bilingual skills in Spanish and/or Asian languages are a plus.
  • Ability to perform public speaking and make professional presentations.
  • Ability to work collaboratively and independently.
  • Proficiency with applications such as Outlook, Word, Excel, PowerPoint, eCW, eICARE, AZARA, HCS, Healthix, MAPP, PSYCKES, etc.
  • Ability to multitask and manage multiple projects in a stressful environment.
  • Flexibility to work some evenings and weekends. Essential Functions and Responsibilities:
    • Ensure compliance with program standards, quality, budget, and reporting requirements.
    • Assist in grants management, including revenue projection and budgeting.
    • Recruit and hire program staff.
    • Provide clinical and administrative supervision to staff.
    • Participate in multidisciplinary meetings.
    • Develop and implement recruitment and enrollment strategies.
    • Support staff to meet quality and quantitative goals.
    • Conduct quality assurance and improvement activities.
    • Maintain educational and promotional materials.
    • Address crises involving clients, medical, and mental health providers.
    • Coordinate with other programs to ensure comprehensive services.
    • Prepare and submit reports timely.
    • Provide training related to Support Services.
    • Participate in organizational activities and represent the agency externally.
