POSITION: Program Coordinator, Shelter Plus CareOFFICE/DIVISION: Shelter Plus Care Program/Community Reinvestment DivisionBULLETIN No: 02082025SALARY RANGE: $62,000-$67,000 (Commensurate with experience)OPENING DATE: August 14, 2025MAJOR DUTIES: Coordinate all aspects of UPO's Shelter Plus Care ( SPC ) program and manage day-to-day program activities. Make monthly home visits to each client, provide case management, and housing inspections. Prepare individualized case plans for each client based upon their need to improve in the areas of wellness, education, employment and self-sufficiency. Update these plans every six months. Prepare monthly reports on home visits. Submit regular reports on program outcomes, including statistics on customers served, customer demographics internally and externally to funders, as requested. BRIEFDESCRIPTION: Conduct Initial Assessment and Service Plan for new clients. Provide ongoing counseling and referrals to achieve goals for self-sufficiency set forth in the service plan, specifically including goals related to education, employment, wellness and housing. Conduct monthly home visits to verify household income and family composition, collect receipts for rent paid, verify status of utilities, verify conditions in the housing accommodation, verify working smoke detectors, interview clients and identify any client concerns. Conduct client satisfaction surveys every 6 months. Attend mandatory meetings and training courses required by The Community Partnership (TCP). Enter program outcome data accurately into both UPO's Empower database and TCP's designated database. Document/update client demographics, contacts, and referrals in CASA. Participate in instructional and curriculum articulation activities with other programs and other divisions. Become well-versed in UPO's vision, mission, and strategic plan goals, and consistently model behaviors that align with these principles. Other duties as assigned. CORE COMPETENCIES:
