Primary Purpose

Responsible for coordination, facilitation, and support of educational and referral services to assist in the prevention of fetal infant mortality.

Education

•Must have a Bachelor's degree in Social Services, or a related field.

•Prefer a Master's degree in a related field.

Experience

•Must have two (2) years of experience in a community health organization.

Equivalent Education and/or Experience

•May have an equivalent combination of education and/or experience in lieu of specific education and/or experience as stated above.

Certification/Registration/Licensure

•None.

•Must have excellent verbal and written communication skills. Must be able to communicate effectively with all levels of staff and management.

•Must be organized and detail oriented.

•Must be able to demonstrate client centered/client valued behavior.

•Must have excellent interpersonal skills, particularly in working with a diverse population.

Responsibilities

1.Assists with the promotion of the Fetal Infant Mortality Review (FIMR) project partnerships. Maintains regular contact with outside agencies and families to establish cooperative relationships and to obtain programmatic information.

2.Assists with the development and presentation of information and materials to educate target group to promote health awareness on FIMR project and good health practices, including health education events. Sends introductory letters out to all applicable families.

3.Builds and maintains a collaborative relationship with the FIMR Case Review Team and the Community Action Team, maintaining meeting reports/summaries.

4.Promotes consumer participation in FIMR related activities, including active membership of consumers on Community Action Team and peer involvement in community based agencies.

5.During FIMR Case Review Team meetings, responsible for completing Case Review Issues Summary. Participates in writing quarterly and annual reports on FIMR findings.

6.Refers participants and families to other specialized agencies for additional assistance and/or treatment. Assists the Community Action Team to develop specific intervention methods for reducing infant mortality.

7.Maintains a positive working relationship with management, vendors, community agencies, schools, other health educators and healthcare personnel to ensure open communication within the health community.

8.Assists with the evaluation and revision of literature and materials periodically to ensure that current information is disseminated to participants, community stakeholders, and staff.

Job Accountabilities

  • Identifies ways to improve work processes and improve customer satisfaction. Makes recommendations to supervisor, implements, and monitors results as appropriate in support of the overall goals of the department and Parkland.
  • Stays abreast of the latest developments, advancements, and trends in the field by attending seminars/workshops, reading professional journals, actively participating in professional organizations, and/or maintaining certification or licensure. Integrates knowledge gained into current work practices.
  • Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the area. Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans. Seeks advice and guidance as needed to ensure proper understanding.
