Summary The Program Coordinator for the Clinical Rapid Response Team (CRRT) will be responsible for overseeing the daily operations, coordination, and implementation of the program. This role involves managing logistical and operational aspects of the program, ensuring effective communication across team members, and coordinating services to support high acuity clients. The Program Coordinator will collaborate closely with clinical staff, peer specialists, and external stakeholders to ensure seamless service delivery and optimal client outcomes. The ideal candidate will possess strong organizational, communication, and problem-solving skills, and a commitment to person-centered care. This is a new team to help address existing gaps in the overall behavioral health housing system. Reporting to the Chief Program Officer and Medical Director, the Program Coordinator oversees the administrative functions of the program and provides services, ensuring the ongoing best practice approach of program functioning, compliance with regulatory agency policies and procedures, and a proactive approach to crisis. ResponsibilitiesProgram/Facilities

  • Assists in designing, planning, administering and evaluating the response team to ensure compliance with Urban Pathways, regulatory and funding agency regulations policies and procedures.
  • Manages and ensures that comprehensive services are provided to high acuity clients under the care of Urban Pathways.
  • Provides leadership to program staff around the care of high acuity clients
  • Manages and develops staff, including coaching training and mentoring.
  • Meets weekly with all direct reports for individual supervision to monitor and support administrative goals and functions.
  • Provides direct services to some of the Tier 1 high acuity clients
  • Participates in recruiting, hiring, training, and evaluation of staff.
  • Conducts field work, including, but not limited to program visits, outreach, and community-based case conferences.
  • Locates, establishes, and maintains all community resources necessary to operate the program.
  • Oversees all quality improvement monitoring functions including, but not limited to 360 case reviews, Utilization Reviews, and Service Plan reviews.
  • Reviews all case records to ensure quality improvement and compliance with Urban Pathways, funding and regulatory agencies, including incident reporting and investigation documentation.
  • Coordinates clinical meetings with staff and other inter-disciplinary teams.
  • Participates in the development of Urban Pathways program policies and procedures.
  • Liaises /coordinates services with psychiatric and medical providers.
  • Under the leadership of the Medical Director and the Chief Program Officer, helps to oversee/monitor clinical provisions of the program to ensure they align with best and evidenced based practices, including but not limited to casework, case plans, housing placements, substance use, incident reporting and investigation documentation and all other activities.
  • Reviews all case records to ensure quality improvement and compliance with Urban Pathways, funding and regulatory agencies.
  • training sessions to enhance staff skills and knowledge in crisis intervention and trauma-informed care.
  • Available to address emergency needs on a twenty-four hour basis.
  • Administrative
  • Prepares, maintains and submits timely and accurate statistical and other reports mandated by Urban Pathways, regulatory and funding agencies.
  • Assesses, coordinates, and facilitates staff training.
  • Monitors and ensures accuracy of employee payroll system.
  • Ensures timely evaluations of program staff, identifying areas for improvement and corrective actions as needed.
  • Collaborates with HR to recruit, hire and onboard new program staff.
  • Manages AWARDS and other systems to ensure that all necessary person served data is accurate and timely.
  • Ensures paper and electronic charts comply with Urban Pathways and regulatory and funding agency policies and procedures.
  • Maintains staffing schedule according to program needs and funding requirements.
  • Dispatches the team according to program needs. Needs can include trainings, engagement events and crises.
  • Assist in the development and implementation of quality assurance initiatives, including client satisfaction surveys, feedback collection, and performance reviews.
  • Assist in the preparation of program proposals, grants, and other documentation required for funding and resource allocation.
  • Manage program-related administrative tasks, including budgeting, invoicing, and procurement of materials and supplies.
  • Ensure adherence to policies and procedures, promoting accountability and high standards of service delivery.
  • Community Relations
    • Develops and maintains ongoing relationships with community services and resources.
    Communication
    • Communicates verbally and in writing in a professional manner
    • Facilitates/attends meetings, trainings, and supervisory sessions.
    • Responds to all communications including telephone, emails, and other oral and written inquiries in a timely manner.
    Performs all other duties as assigned and as required.QualificationsEducation and Experience
    • Master's degree in Social Work or closely related human service field required.
    • Professional licensure (e.g. LMSW) preferred.
    • Minimum of 3 years of clinical and supervisory experience required.
    • BSW with 5 years related experience and CASAC considered.
    • Experience with working with people who carry a diagnosis
    Skills
    • Strong written and verbal communication skills.
    • Strong team building and coaching skills and experience.
    • Strong problem-solving skills and resourcefulness.
    • Strong computer skills and knowledge of Microsoft Office (Word, Outlook, Excel, etc.).
    • Strong skills and knowledge of client relational databases (e.g. Awards preferred).
    • Foreign languages a plus.
    Other
    • Required to work a flexible schedule including evenings and weekends.
    Licenses
    • Valid driver's license with clean driving record a plus.
    Physical Requirements
    • All programs require field work, home visits, client escorts, and use of public transportation.
    • Ability to sit, stand, walk, bend, stoop, push, pull, lift (up to 25 lbs.), type, write, read, speak, see, and hear.
