The Brooklyn Transitions Family Treatment/ Rehabilitation (FT/ R) program is a specialized, home-based, borough-wide prevention program utilizing the Solution-Based Casework (SBC) model for families facing child welfare-related challenges due to substance abuse and/or mental health concerns who want to repair and improve family relationships and ensure the safety and well-being of children so that families can safely stay together. The Program Coordinator is responsible for the management and implementation of various program operations and functions that comprise an effective and successful program. The Program Coordinator is a key program leader and collaborates closely with the Program Director to ensure the program exceeds contractual commitments set forth by NYC Administration for Children s Services (ACS).
