POSITION: Program Coordinator - Agency With Choice Full Time Position The Arc of Chester County is a non-profit organization dedicated to advocate, educate and provides services to empower individuals with disabilities and their families to enhance the quality of their lives. POSITION SUMMARY The Agency with Choice Program (AWC) is a service that makes it possible for people with intellectual and developmental disabilities to self-direct the supports they need in order to remain at home or live independently in the community of their choice. The Program Coordinators:
- Establish and maintain a professional partnership with both internal and external stakeholders including participants, families, volunteers, Support Service Professionals, teams and etc.
- Prepare documentation for ISP meeting and team meetings.
- Attend annual ISP meetings and other team meetings in the participant's home and/or community, as required.
- Provide and document onboarding, annual and ongoing training to the participant's volunteers and staff.
- Collect records and review progress notes and service notes for accuracy and completion, and submit for filing or corrective action.
- Verify and approve services rendered in accordance with billing requirements.
- Oversight and approval of staff time worked in accordance with Electronic Visit Verification compliancy.
- Track service utilization as outlined in the ISP to document, monitor and ensure the utilization of all services is occurring as written in the ISP.
- Serve as Incident Management point person for assigned participants.
REQUIREMENTS: - Minimum of Associate's degree, and at least 2 years working in customer service/ human services position.
- Current and valid driver's license & driving record as per The Arc's Safety Policies and Procedures.
- Excellent organizational, interpersonal, communication and writing skills required.
- Ability to work independently, to set priorities and follow through on details.
- Strong written and oral communication skills.
- Strong computer skills and technology skills.
- Experience coordinating and working with volunteers.
- Ability to problem-solve and to identify issues and concerns that may arise.
- Capable of gathering all required data collection pertaining to issues/concerns and service provision.
