The Rogosin Institute (RI) is an independent not-for-profit medical care and research organization affiliated with NewYork Presbyterian Hospital and Weill Cornell Medicine. Rogosin is one of the premier centers for the diagnosis and management of kidney disease in the country. Rogosin treats patients with diabetes, hypertension, lipid disorders and conducts research in these areas.
Rogosin's Prevention and Education in Advanced Kidney Disease (PEAK) Program is looking for an experienced Program Administrator who has a background in healthcare, public policy, and/ or ccommunity outreach. PEAK is dedicated to improving the lives of patients with late-stage chronic kidney disease (CKD) through education and patient empowerment with health care choices. The program provides education regarding the trajectory of kidney disease with a focus on individualized care plans, lifestyle choices and has shown to have improved clinical outcomes. PEAK aims to create new low-cost models for the prevention, and improvement of care, for individuals with CKD and end-stage renal disease (ESRD). The PEAK program consists of a multi-disciplinary care team, including social workers, dietitians, nurse navigators, and community nephrologists. The program initially established on the Upper Eastside in NYC, is now expanding. The Program Administrator will be responsible for expanding the model into Washington Heights and a large footprint in Brooklyn. The Upper Westside of NYC will serve as a corporate headquarter.
Job duties include but are not limited to ensuring the implementation of strategic priorities, including the building, and evaluation of this model of urban healthcare while mobilizing communities to assume more responsibility for their own health. The individual interested in this position must be skilled in in the development of programs budgets and community integration. This person would work to broaden CKD education and establish a larger base for patient resources. Leadership skills, community-based research, and coordinating information about the PEAK work for multiple audiences will be play a critical role in program development. S/he will assist with program development, implementation, operation, and evaluation of PEAK's Programs including community programming and health promotion efforts.
