SummaryJob Description:Referral RN - Palliative CarePRN - as needed/variance in scheduleJob Description:The Palliative Care Referral RN will work with the Palliative Care team to support clinical staff and physicians in the management of the continuum of care for palliative care patients. This includes assisting with patient evaluation, level of care determination, patient management, and goals of care conversations. The RN will assist with the education of staff and physicians.
