More about our teamOur Population Health team works in partnership with primary care practices to improve patient outcomes, support care gap closure, and drive clinical quality improvements. We focus on patient advocacy, provider support, and implementing innovative strategies to manage wellness, preventive screenings, and chronic disease management in the ambulatory care setting.
How you'll contributeA Population Health Registered Nurse (RN) who excels in this role:
Provides onsite support in primary care clinics for Annual Wellness Visits, patient care gap closures, disease-specific education, and social determinants of health screenings.
Conducts pre-visit chart reviews to identify needed preventive care, immunizations, and chronic disease interventions.
Works independently to assess patient understanding of disease processes, treatment plans, and lifestyle changes while applying professional nursing skills.
Educates patients and families, establishing mutual short- and long-term goals for care, including medication adherence, fall prevention, and nutrition.
Identifies patients with high-risk conditions and collaborates with the care team (primary care provider, care navigator, community health worker, etc.) to address barriers to care.
Utilizes the electronic health record (EHR) to document diagnoses, close quality measures, and train clinical staff in population health initiatives.
Reviews monthly and quarterly reports on quality and utilization to identify performance improvement opportunities.

Applicants should have a current RN license and prior experience in an ambulatory or primary care setting. Additional requirements include:
Strong critical thinking, clinical judgment, and patient education skills.
Ability to work independently and collaboratively as part of an interdisciplinary care team.
Experience with electronic health records (EHRs) and data analysis for clinical quality measures.
Knowledge of chronic disease management, preventive care, and patient engagement strategies.
Effective communication skills with patients, families, and providers.
Travel Requirement: Some local travel between primary care clinics may be required.
