Population Health Patient Care Partner - Ardent Health Services : Job Details

Conduct telephonic or electronic outreach to patients identified for care management services. Explain the purpose, benefits, expected copay/coinsurance, and expectations of the care management program in a clear and compassionate manner. Obtain and document verbal consent for participation in the care management program. Complete standardized initial screenings as part of the enrollment process. Accurately document all patient interactions and screening results in the electronic health record (EHR) care management platform. Collaborate with care managers, nurses, and other team members to ensure smooth handoff and continuity of care.
