Job Description :
Lead data-informed initiatives to enhance population health outcomes and reduce disparities. Monitor clinical quality metrics (HEDIS, QARR, UDS), identify care gaps, and support alignment with national and state benchmarks. Oversee quality improvement and risk management programs using tools like Plan-Do-Study-Act (PDSA). Manage pay-for-performance and shared savings programs, including contract oversight and performance reporting. Provide strategic oversight of patient attribution, panel management, and incentive program participation. Chair or co-chair internal committees focused on quality, risk, and population health. Supervise staff and ensure coordination across departments for training, compliance, and clinical performance. Support operational oversight of Health Home programs through September 2025.
