Job Description :

Under the direction of the Director, the Population Health Manager uses a data-driven approach to support Quality and Performance Improvement program activities and guide population health and value-based initiatives in Ambulatory Care. This position collaborates with other stakeholders to gather data, run analytic reports, and contribute to the analysis and interpretation of results. This includes reviewing patient charts to identify gaps in care, and developing reports and analytics to support frontline staff and clinical operations. They implement Population Health projects and supervise assigned team members.

  • Collaborate with leadership, practice management, IT, and other stakeholders to collect and analyze data to improve the quality of clinical care.
  • Implements and manages assigned programs, establishing strategic direction, program goals & metrics, monitoring progress, and managing relationships.
  • Provides direction, support and supervision to assigned team members, volunteers, and interns. Holds meetings to keep team members up to date with policy and procedures, and to address any issues. Provides oversight, instruction, and training as necessary. Manages and enters E-time.
  • Produce and distribute pre-visit planning reports to clinical and patient-facing teams to guide patient care and improve care quality and performance.
  • Lead daily, weekly, and monthly data-management tasks.
  • Run dashboards, reports, and data analysis using data extracts from a range of sources.
  • Review electronic medical records to confirm if patients are missing services, and develop reports for scheduling, or a supplemental data report where care has already been provided.
  • Develop presentations, diagrams, reports and flowcharts.
  • Establish and maintain strong relationships with key stakeholders and partners, building commitment to shared goals and program activities.
  • Develop and maintain work plans including key deliverables and timelines for major milestones. Ensure timelines are met or changes are communicated to appropriate stakeholders.
  • Participate in quality improvement activities, meetings and committees.
  • Other duties as assigned by the Director.
Education:
  • Bachelor's Degree with 2 years of healthcare experience required.
  • Masters of Public Health Degree preferred
License: None Experience/Skills Required
  • Project management experience with demonstrated ability to create buy-in across multiple, diverse team members.
  • Ability to work collaboratively with clinical and operational leaders and staff to effect change.
  • Experience in managed care, population health, and value-based care required.
  • Experience creating, manipulating, and analyzing data sets, and reporting findings is required.
  • Candidates must have advanced skills with Microsoft Excel, and be proficient at vlookup, pivot tables and formulas.
