Specialty State CA Location 1165 Montgomery Dr., Santa Rosa, CA 95405 Requirements LOCUM- General Surgery PA Please ensure all bids reflect the following: - Guaranteed Hours: 8 hours per day - OT applies after 8 hours per day We are looking for one full time provider to work 8 hours a day/ 5 days a week. NO OT is expected. Please seek preapproval for any OT with the medical director. There will be 5 surgeries a dayPotential for weekend call (12 or 24 hour). OR experience requiredACTIVE CA License and ACTIVE CA DEA required. Must have general surgery experience. Any PAs must be certified (PA-C). Please note that privileging timeline will not begin until the RFA and application are turned back in. Typical turn around for RFA is 48-72 hours. Application should be submitted within 5 days. Update 7/31: oAll HBS specialties require 2 years of current Hospital clinical activity in the specialty.oGaps greater than 3 months must be explained.oInclude current number of case logs upon presentation for surgical specialties.
