Fort HealthCare is a patient-centered hospital and healthcare system serving individuals, businesses and families in Jefferson, Dane, and Walworth Counties, and we are seeking a Physical Therapist to join our Whitewater Clinic team. The Physical Therapist provides physical therapy evaluation and treatment to patients with musculoskeletal and/or neurological impairments. Also responsible for the monitoring and documentation of patient status, treatment and response. Responsibilities of this position include:

• Performs physical therapy evaluations and develops a plan of care that is appropriate for the diagnosis and age of patient.
• Performs physical therapy patient treatment as per the plan of care and modifies treatment plan based on ongoing assessment of patient condition.
• Demonstrates an understanding of the operation and indications for commonly used modalities and equipment.
• Documents evaluations, daily notes, progress notes, and discharge notes in a timely manner; therapy charges are supported by this documentation.
• Communicates and documents necessary information appropriately with patients, families, physicians, and interdisciplinary team members.
• Performs other duties as assigned.
• Completed department-specific orientation and annual competencies for this position.

Requirements for this position include:

• Bachelors Degree required.
• Masters Degree preferred.
• Works well under pressure preferred.
• BLS certification, valid Wisconsin driver's license, license to practice physical therapy in Wisconsin required.
