Job Title: Physical Therapist (PT) Location: South Dennis, MA Type: Contract Duration: 13 Weeks Pay Rate: $55 - $62 per hour Responsibilities:

• Conduct comprehensive physical therapy assessments, considering physiological, psychosocial, and environmental factors.
• Develop and implement personalized plans of care based on assessment findings.
• Maintain Home Health Resource Group (HHRG) values in accordance with national standards.
• Monitor patient outcomes and ensure benchmarks are met annually.
• Refer patients to community resources when appropriate.
• Regularly update care plans and document changes accurately.
• Participate in interdisciplinary case conferences to coordinate care effectively.
• Supervise Home Health Aide plans every 14 days and maintain documentation.
• Notify physicians and care team of significant changes in patient conditions.
• Complete all admission and discharge documentation timely, ensuring coordinated discharge planning.
• Demonstrate service excellence consistent with core values: compassion, accountability, respect, excellence, and service.

Qualifications:

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Active Physical Therapist license in the state of Massachusetts.
• Minimum of 1 year of clinical experience.
• Current CPR certification.
• Proficiency with electronic documentation systems is a plus.
