Physical Therapist- PT Home Health - Woodlands Home Health : Job Details

Woodlands Home Health has the oppurtunity for you to join their team as a Home Health Physical Therapist! This is a traveling positon with company paid mileage from first visit to last visit, or the oppurtunity to use a company car for travel to patient visits. Have the freedom to create your own schedule, want to start at 8am? want to start at 9am? you deciede! Work with their amazing PTA's to share caseload Job Duties include but are not limited to:

• Evaluate and assess patients' physical conditions and develop appropriate treatment plans
• Administer therapeutic exercises, manual therapy techniques, and modalities to improve patients' mobility, strength, and function
• Monitor patients' progress and modify treatment plans as necessary
• Educate patients and their families on proper techniques for managing their conditions and preventing further injury
• Collaborate with other healthcare professionals to coordinate patient care and ensure optimal outcomes
• Maintain accurate and up-to-date patient records

Education & Requirements:

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program
• Must have an active Physical Therapy license in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.
