Job Title: Physical Therapist Professional

Location: Mantey Heights in Grand Junction, Colorado

Job Type: Full-Time/Part-Time

Reports To: Director of Rehabilitation or Therapy Services Manager

Job Summary:

The Physical Therapist is integral to enhancing the well-being of residents in our skilled nursing facility. You will evaluate patients, create tailored treatment plans, and deliver compassionate physical therapy aimed at boosting their strength, mobility, and overall quality of life. We prioritize compliance with all federal, state, and facility regulations to ensure the best care possible.

Essential Duties and Responsibilities:

  • Conduct comprehensive assessments of residents' physical abilities and mobility.
  • Craft, execute, and periodically adjust individualized treatment plans based on assessment findings.
  • Provide professional physical therapy treatments following established best practices.
  • Educate residents, families, and staff on effective techniques and home exercise programs.
  • Maintain thorough documentation of evaluations, treatment sessions, progress notes, and discharge summaries.
  • Collaborate with an interdisciplinary care team, actively participating in care conferences and discharge planning.
  • Supervise and guide Physical Therapist Assistants (PTAs) in accordance with Colorado licensure laws.
  • Ensure adherence to Medicare, Medicaid, and facility documentation and billing standards.
  • Maintain cleanliness and safety of equipment and therapy areas.

Qualifications:

  • Doctorate or Master's degree in Physical Therapy from an accredited institution.
  • Current Colorado Physical Therapist License in good standing.
  • Valid CPR certification.
  • Prior experience in skilled nursing, long-term care, or geriatric rehabilitation is preferred.
  • Exceptional interpersonal, organizational, and documentation skills.
  • A collaborative spirit to thrive in a team-oriented, resident-focused environment.

Physical Requirements:

  • Ability to lift up to 50 pounds.
  • Frequent standing, walking, reaching, and assisting residents with transfers or ambulation.
  • Capability to perform tasks requiring physical stamina and mobility in a clinical setting.
