- Conduct initial assessments using appropriate tools based on patient age and developmental level.
- Develop individualized treatment plans with measurable goals and select interventions suitable for the patient's age and abilities.
- Evaluate patient response to treatment, amending treatment plans as needed and providing updates on patient status to families and healthcare teams.
- Document clinical progress and interventions in accordance with policy, including daily notes and discharge summaries.
- Provide education to patients and families to support continued functional independence and integrate them into treatment and discharge planning.
- Maintain professional growth and development through ongoing education and attending professional workshops.
- Adhere to all applicable federal and state guidelines and maintain a high level of professionalism within the clinical setting.
Minimum Position Qualifications:
- Bachelor's degree in Physical Therapy from an accredited program.
- Current Physical Therapist licensure in the state of New York.
- Mandatory BLS certification.
- Proven experience in a physical therapy role, with a strong understanding of various treatment modalities.
- Excellent communication and documentation skills.
