Physical Therapist - STI : Job Details

Health Advocates Network is currently seeking a Physical Therapist to work at a facility in Orangeburg, NY. These are registry positions with our company. $100 Sign-On Bonus available. Pay Rate: $60 / hour. Employment Type: W2.

• Act as physical therapy consultant.
• Assess and evaluate client's condition to develop a physical therapy treatment plan based upon physician's request.
• Review client's treatment goals, treatment, and medical history.
• Perform and interpret standard muscle, nerve, joint and functional ability tests.
• Administer appropriate physical therapy treatments when indicated.
• Prepare documentation of assessment, evaluation, delivery of treatment services and patient progress for the client's medical record.
• Consult with client's treatment team as needed.
• Be supervised by RPC's Director of Medicine.

Shift: Days, Monday - Friday. The contractor must be flexible to meet the needs of patients within RPC and in various outpatient clinics as needed and approved by the Medical Director.

Contractor must be able to provide coverage when primary physical therapists are unavailable due to illness, vacation, etc.

• Contractor must supply current New York State licensed physical therapists.
• Designated staff must possess, at no cost to the STATE, all qualifications, licenses, and permits to perform physical therapy services; contractor must engage in the business of physical therapy consultant services as required within the jurisdiction (state & local) where the specified service is to be performed, comply with all state and local regulations governing physical therapy consultant services.
• Contractor must have at least four (4) years of satisfactory experience in providing physical therapy consultant services to mentally ill clients who may be directly post-operative.
• Contractor shall submit a list of at least four (4) references for whom contractor has provided like or similar services within the past five years.
• Contractor is expected to provide only qualified and experienced staff for this contract. Rockland Psychiatric Center reserves the right to investigate qualifications of all staff assigned to provide services to RPC patients to determine their ability to properly perform the services being sought. • Contractor is expected to have privilege and credentials by Medical Services Office (MSO) every two years and must participate in the annually peer review.
