Physical Therapist – Skilled Nursing Facility

As a Physical Therapist, you'll play a key role in optimizing mobility and improving quality of life for adult and geriatric patients. You'll focus on rehabilitation and wound care in collaboration with an interdisciplinary team.

📍 Location: Contra Costa County, CA

💼 Employment Type: Full time

🕗 Schedule: 8:00 AM – 4:00pm or 9:00 AM - 5:00 PM (M–F, Sun–Thurs, or Tues–Sat)

💰 Salary: $124,800 – $135,200 annually

🏥 Setting: 5-Star Medicare Skilled Nursing Facility

📅 Experience Level: New grads welcome! Mentorship available

Responsibilities:

• Conduct assessments and develop individualized treatment plans
• Implement evidence-based wound care techniques
• Educate patients, families, and staff on wound prevention and rehab strategies
• Accurately document care using NetHealth EMR
• Ensure compliance with Medicare, Medicaid, and CA state guidelines
• Maintain patient confidentiality in line with HIPAA regulations

Requirements

• California Physical Therapist License (active & in good standing) – Required
• BLS Certification – American Heart Association or Red Cross
• 1+ years of Physical Therapy experience preferred (SNF or wound care ideal)
• Knowledge of Medicare/Medicaid therapy documentation standards
