Evaluates patients presenting or referred to physical therapy by completing initial evaluations within established timeframes. Applies appropriate diagnostic and prognostic testing for musculoskeletal, neurological, and neuromuscular conditions. Utilizes standardized testing to determine the level of disability and track outcomes throughout the patient care experience. Performs a variety of best practice and evidence-based therapeutic interventions tailored to meet the unique rehabilitation needs of each patient. Establishes treatment plans with measurable and attainable goals collaboratively with patients, families, and other healthcare team members, including considerations for discharge planning. Selects and implements therapy activities per the plan of care to ensure that patient, family, and therapy goals are met. Recognizes individual limitations and capabilities and effectively utilizes available resources. Evaluates patients' learning styles and teaches them the skills and techniques necessary for participating in physical therapy. Consults with rehabilitation team members to select the most appropriate activities and programs for individual patients based on their needs and capabilities. Develops treatment plans including specific goals and activities, reassessing and modifying plans as necessary based on the patient's current level of function. Communicates effectively with patients, families, staff, physicians, and other stakeholders regarding physical medicine and rehabilitation services, maintaining confidentiality at all times. Documents all patient care activities including evaluations, progress notes, and detailed treatment records according to standards of practice and regulatory requirements. Facilitates the preparation of patient schedules, billing charges, and other reports while adhering to medical records procedures. Utilizes descriptive, behavioral terms with correct grammar, organizationally approved abbreviations, and medical terminology in all documentation. Documents ongoing status, interventions, patient responses, and outcomes to ensure accurate billing. Participates in personal, professional, and program development by attending in-services and meetings, sharing expertise to enhance service quality and staff development. Assists in training physical therapy students and other personnel while acting as a resource for new staff members and coworkers. Adheres to the professional code of ethics and meets all accreditation requirements. Demonstrates effective time management and proactively prioritizes workload to meet productivity requirements.
