Physical Therapist

Location: Taconic Rehabilitation and Nursing at Hopewell

SHIFT: Full Time and Per Diem

JOB DESCRIPTION:

Physical Therapist Floats work with patients to improve their movement and manage their pain. PTs use a variety of techniques to help their patients, including hands-on therapy, strengthening and stretching exercises, electrical stimulation, ultrasound, ice/heat, and much more.

RESPONSIBILITIES:

  • Conducting physical and psychological assessments of clients to understand their physical condition and developing or following a treatment plan.

  • Participate in family care plan meetings and educate caregivers and family members of clients on patient care

  • Creating personalized fitness-oriented health care programs for patients.

  • Administer medically prescribed physical therapy treatments to relieve pain and improve mobility.

  • Consult and collaborate with other healthcare professionals regarding patient.

  • Documenting patient care history.

  • Demonstrate ability to work with geriatric residents

  • Other duties as assigned

REQUIREMENTS:

  • New York State Physical Therapist licensure

  • Degree in Physical Therapy from a program accredited by the American Physical Therapy Associtation (APTA) required

  • CPR certification

  • Physical strength to lift between 50-100lbs

  • Must have knowledge, skills, and techniques of age specific developmental factors and be able to evaluate patients

  • Must have up-to-date knowledge for treatment practices and physical therapy programs

  • Experience in long term care preferred

  • Must be flexible, punctual, professional, and reliable

  • Strong customer service skills and compassion

  • Able to collaborate with a wide range of health care disciplines, as well as offer insight to patient and family members

  • Strong written and verbal communication skills

  • Respect for residents' privacy and rights; display of genuine concern and empathy for the elderly

  • Able to perform tasks while upholding high standards of quality, cleanliness, and kindness
