The Physical Therapist is responsible for evaluating, planning, and implementing therapy services to patients in a Skilled Nursing Facility (SNF) setting. The ideal candidate must possess recent and relevant experience in an SNF and hold an active state licensure in physical therapy.

• Licensure: Active and unrestricted Physical Therapist license in the state of practice.

• Experience: Minimum 1 year of recent experience in a Skilled Nursing Facility (SNF) required.

• Education: Graduate of an accredited Physical Therapy program.

• Certifications: CPR/BLS certification (required); additional rehab certifications (preferred).

• Skills: Strong clinical judgment, effective communication, EMR proficiency, and patient-centered approach.
