Job Summary:

The Physical Therapist provides home care services across various locations, ensuring timely assessments, individualized treatment, and accurate documentation. The therapist will coordinate care with an interdisciplinary team, support patient goals, and uphold the highest standards of service delivery and compliance.

Job Responsibilities:

• Perform comprehensive evaluations using standardized assessment tools.
• Design and implement personalized therapy plans based on clinical findings.
• Administer therapeutic interventions, adjusting care as needed.
• Maintain timely and accurate electronic documentation (within 2 business days).
• Collaborate with the care team in case conferences and ongoing communication.
• Educate patients and families on treatment, safety, and functional goals.
• Assist with discharge planning and coordinate equipment as needed.
• Supervise Physical Therapist Assistants (PTAs) during treatment implementation.
• Participate in continuing education and quality improvement initiatives.

Required Skills & Experience:

• Prior experience in a medical facility or home health setting.
• Independent, self-directed work ethic with strong communication skills.

Preferred:

• Experience in hospital-based or home care physical therapy.

Education:

Required:

• Bachelor's and/or Master's Degree in Physical Therapy from an accredited program.

Preferred:

• Doctorate in Physical Therapy.

Licensure & Certification:

• Required:
• Active New York State Physical Therapist License
• Current BLS Certification (AHA, HSI, or American Red Cross)

Assignment Details:

• Start Date: August 4, 2025
• Contract Duration: 13 weeks
• Schedule: Monday – Friday, 8:00 AM – 5:00 PM
• Hours per Week: 40
• Note: Candidate must have home care PT experience. Travel is required to various patient residences.
