Position Summary
The Physical Therapist is responsible for providing high-quality, individualized patient care through comprehensive assessments, treatment planning, and implementation of physical therapy programs. The ideal candidate is a compassionate professional who collaborates well with interdisciplinary teams and is committed to helping patients reach their functional goals.
Key Responsibilities
- Assess patient conditions and develop individualized treatment plans and goals.
- Appropriately code procedures and physical therapy diagnoses.
- Verify insurance authorizations prior to evaluations or treatment.
- Document all patient interactions, assessments, and treatment plans using E-ClinicalWorks EMR.
- Gain competency in EMR usage; seek support when needed.
- Supervise Physical Therapist Assistants when applicable.
- Operate therapeutic and exercise equipment properly and safely.
- Collaborate with physicians and healthcare providers to ensure coordinated patient care.
- Adhere to CMS and MIPS reporting standards.
- Educate patients and families on therapy plans and home exercises.
- Serve as a knowledge resource for colleagues and patients.
- Perform additional duties as agreed upon with leadership.
Qualifications
- Valid Oregon Physical Therapist License or eligibility to obtain one.
- Current BLS certification required.
- Experience or knowledge of FQHC operations is highly preferred.
- Strong interpersonal skills with patients, providers, and staff.
- Ability to work independently and as part of a collaborative team.
- Professional, patient-centered approach with a commitment to confidentiality.
- Flexible availability, including occasional evenings and weekends.
- Must not be listed on any State or Federal exclusion list.
Physical Requirements
- Ability to stand and walk for extended periods.
- Capable of lifting/moving up to 25 pounds.
- Frequent bending, reaching, kneeling, and crouching.
- Regular use of hands, hearing, speaking, and visual abilities.
