Job Description :
Sign-On Bonus: Up to $20000 based on experience. Shift: Days Office Hours: 08:30 AM to 05:00 PM Schedule: Monday to Friday Key Result Areas Quality:
  • The employee demonstrates accuracy, and reliability, and assumes responsibility for the quality of work performed.
  • Follows District rules and regulations maintaining appropriate licensure, certification, and authorization required for employment.
  • Follows all infection control regulations and uses standard precautions appropriately.
  • Uses proper body mechanics and safety equipment.
  • Performs duties in a safe manner.
  • Follows all safety, fire and disaster regulations.
  • Is knowledgeable about hazardous materials and Material Safety Data Sheets (MSDS) for chemicals used in the work area.
  • Reports safety hazards and injuries immediately to supervisor.

  • The employee works independently and requests assistance/direction when needed.
  • ssumes responsibility for a clear and orderly work area.
  • Dresses appropriately for work assignment as per dress code.

  • The employee makes good use of time and resources, and completes works assignments.

Primary Duties:
  • Maintain established departmental policies and procedures, objectives, qualify improvement program, safety, environmental and infection control standards.
  • Evaluate referred patients and develop treatment plans including specific goals and activities, reevaluating and modifying plans as necessary.
  • Provide instructions in therapeutic procedures to be continued by the patient.
  • Provide direct physical therapy to patients per plan of care.
  • Maintain active and support communication with the patient/caregiver regarding progress, problems, home programs and other issues related to the therapeutic process.
  • Prepare written documentation as required by the profession and the department such as evaluation results, individualized treatment plans and progress reports.
  • Communicate observations, facts and comments with the interdisciplinary team.
  • ttend patient care conferences and other related meetings.
  • ssist in the Quality Improvement Program and in providing in services to other staff.
  • Enhance professional growth and development through participation in education programs, current literature, workshops and in service meetings.
  • Demonstrate sensitivity to the needs of all cultural and age groups, predominantly the geriatric and dying patient.
Basic Qualifications:
  • Bachelor's Degree in Physical Therapy.
  • 2 year's experience in physical therapy.
  • CPR Certification.
  • California Physical Therapy License.
  • Valid CA Driver license.
  • Continuous automobile insurance coverage.
Preferred Qualifications:
  • Home Health Experience.
  • Prolonged, extensive or considerable standing/walking.
  • Lifts, positions, pushes and/or transfers patients.
  • Lifts supplies/equipment.
