PACE North is looking for a dedicated and compassionate Physical Therapist to lead and deliver high-quality care to the aging population - both at our Center and in participants' homes. What You'll Do: As a Physical Therapist at PACE North, you'll be responsible for helping participants maximize their mobility and independence. You'll:
- Conduct initial and periodic assessments of participants' physical and functional status
- Develop personalized care and treatment plans, including therapeutic exercise, gait training, and mobility support
- Collaborate with an interdisciplinary care team and communicate changes in participant condition
- Recommend and evaluate the need for durable medical equipment
- Train family members, caregivers, and staff on implementing therapy strategies
- Contribute to participant discharge planning and continuity of care across settings
- Oversee Physical Therapy Assistant

Requirements What We're Looking For:
- Education:
- Bachelor's in Physical Therapy (prior to 2000), Master's (2000 or later), or Doctorate
- Licensure/Certifications:
- Licensed Physical Therapist in Michigan
- CPR Certification (We can train)
- Valid Michigan Driver's License
- Experience:
- At least 1 year of experience working with elderly or frail populations preferred
- Skills & Qualities:
- Strong clinical competency in geriatric care
- Basic computer proficiency
- Ability to work independently and collaboratively
- Clear communication and documentation skills
- Commitment to participant-centered care and quality improvement
