PHYSICAL THERAPIST FLOAT Travel required to the following facilities: Highland, Beacon, Hopewell Job Description: Taconic Nursing & Rehabilitation Center is seeking a compassionate and motivated Physical Therapist (PT) to join our team and support our 3 local facilities. As a key member of our Taconic Healthcare Facility skilled nursing and rehabilitation centers, you'll help residents regain mobility, improve strength, and enhance their overall quality of life through personalized therapy plans. Key Responsibilities:

• Develop Treatment Plans: Evaluate residents and create customized therapy programs to meet their rehabilitation goals.
• Therapeutic Interventions: Provide hands-on physical therapy sessions focusing on mobility, strength, balance, and pain management.
• Monitor Progress: Regularly assess and adjust treatment plans based on resident progress and outcomes.
• Collaborate with Care Teams: Work closely with nursing staff, occupational therapists, and physicians to ensure comprehensive care.
• Documentation: Maintain accurate records of treatments, progress notes, and resident outcomes in accordance with facility and regulatory standards.

Qualifications:

• Licensed Physical Therapist (PT) in NYS
• Degree in Physical Therapy from an accredited program.
• Experience in skilled nursing, rehabilitation, or long-term care (preferred).
• Strong interpersonal, communication, and organizational skills.
• Ability to motivate and inspire residents to achieve their goals.
