Title: Pharmacist

Location: Watertown, NY

Duration: Permanent

Shift - 8-hour Rotating (Days, Evening, Occasional Nights)

Pay Range - $105,206.40 - $121,971.20

Job responsibilities:

• Participates in interdisciplinary patient care rounds, collaborating with doctors, nurses, and other healthcare professionals, to help maximize patient outcomes.
• Compares patients' medication history obtained against inpatient orders to ensure accuracy.
• Ensures medication regimens are appropriate and follows hospital established policies and evidence-based guidelines. This includes, but is not limited to, antimicrobial use guidelines, renal dosing, and serum drug concentration protocols.
• Reviews all medication orders for appropriate dose, duration, frequency, indication, drug interactions and contraindications.
• Works with medical providers to resolve discrepancies and documents outcomes.
• Serves as a medication expert, providing drug information to clinical staff.
• Compounds sterile intravenous preparations while adhering to USP 797 and USP 800 regulations.
• Ensures compliance with state regulations, federal regulations, joint commission medication management standards, CMS conditions of participation and mentors technician staff to ensure safe and accurate medication handling and dispensing

Licenses & Certification:

• Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school.
• Valid state license to practice as a pharmacist in New York.
• 1 to 2 years hospital experience preferred, not required,
