Overview

The Performance Improvement Change Leader is responsible for applying quality improvement strategies to support projects, products, and programs throughout the organization. The goal is to ensure that the care provided is safe, patient-centered, effective, timely, equitable, and efficient. Under the supervision of Central Health's Quality Improvement Manager, the PICL will work closely with operational and clinical teams to implement, monitor, assess, and improve performance across all PI initiatives. Responsibilities include data analysis, project scheduling, scope management, collaboration, lean activities facilitation, budget monitoring, risk mitigation, community partnership development, and strategic planning. The PICL will lead and facilitate lean activities, support organizational goals through coaching and collaboration, and lead performance improvement initiatives aligned with the Quadruple Aim framework, focusing on clinical effectiveness, population health, performance management, care management, and regulatory reporting.

Responsibilities

PRIMARY RESPONSIBILITIES & DUTIES

  • Lead and support Performance Improvement initiatives using Lean and other quality strategies.
  • Facilitate cross-functional teams to improve organizational performance and pursue strategic goals.
  • Apply project management techniques, such as agile, to support performance improvement projects.
  • Assist in developing and implementing program plans.
  • Teach and share lean tools and practices; promote best practices in performance improvement.
  • Lead performance improvement initiatives in assigned focus areas.
  • Facilitate lean activities like root cause analysis, value-stream mapping, and change management.
  • Coach clinical and administrative staff in sustaining improvement processes.
  • Coordinate with the organization and project teams to ensure aligned efforts.
  • Develop and deliver training, updates, and presentations across the organization.
  • Conduct research and provide summaries to enhance the performance improvement framework.
  • Ensure timely completion of projects and activities.
  • Maintain confidentiality and adhere to HIPAA regulations.
  • Build positive relationships with colleagues and stakeholders.
  • Represent Central Health professionally in all actions and communications.
  • Comply with organizational policies and the mission statement.
Qualifications

Minimum Education

  • Bachelor's degree required.

Preferred Education

  • Master's degree in Health / Business Administration, Public Health, or similar field preferred.

Minimum Experience

  • Experience leading front-line teams through lean/Six Sigma processes in a healthcare setting, including managing complex change and process improvement projects independently.

Required Certification/Licensure

  • Lean Green Belt certification within 6 months of starting the position.
