Job Description:

📍 McHenry County (and Northern Cook County)

🕒 Part Time > 16-24 hours (week days) , 8:00 am to 5:00 pm

🚗 Field-Based

• Monitor vital signs and overall patient condition, reporting changes to the RN or physician

• Provide hands-on personal care: bathing, hygiene, dressing, mobility

• Deliver comfort care measures to maintain dignity and quality of life

• Accurately document care provided, symptoms observed, and patient status

• Offer emotional support to patients and families, fostering trust and reassurance

• Collaborate with RNs, physicians, chaplains, and social workers to deliver coordinated, holistic care

• Active LPN license (state-specific)

• CPR certification (American Heart Association or Red Cross)

• 1+ year experience in acute care or equivalent setting (hospice/home health preferred)

• Strong communication, compassion, and clinical judgment

• Reliable transportation and willingness to travel

• Positive attitude and passion for patient-centered care
